Due to the ongoing COVID-19 pandemic and the rise in new cases, Pure Skin Dermatology will be implementing additional protocols to protect our patients and our staff.

MOBILE APP & REMOTE CHECK-IN PROCESS:
 
Step 1
Download our patient portal app, myPatientVisit  from the Apple App or Google Play store.
 
Note: The myPatientVisit app is available on any Apple or Android device but requires data or internet connection to participate in the virtual waiting room experience depicted below.   
 
Step 2

Log into your myPatientVisit app on your phone. Log in with the username and password you created for your Patient Portal. Click on the blue “I’m here” box & select “Ok”. A staff member will call you when it is time to enter the office.

COMPLETE ALL FORMS:
The Dermatology Online History must be completed to allow check-in. Failure to complete this 48 hours prior to your appointment, will result in cancellation of your appointment.

COME ALONE:
Only scheduled patients may enter the office. Minors may be accompanied by only one
parent/guardian.

MASK UP:
A mask covering your nose and mouth must be worn at all times.

SYMPTOMS/EXPOSURE:
Please reschedule your appointment if you are experiencing any of the symptoms of COVID-19 or have been in contact with someone who has tested positive for COVID-19 within the past 10 days. Symptoms may include fever, cough, shortness of breath, loss of taste and/or smell, sore throat, congestion, nausea, vomiting or diarrhea.

TEMPERATURE CHECKS:
Temperature checks will be done before you enter the office. Your appointment will be
rescheduled if your temperature is 100° F or greater
